(This form authorizes First Presbyterian Church of Saline to obtain MVR and must be completed by the applicant. The church will keep this completed form on file for at least two years after requesting a MVR. This application is to be used with employees and volunteers that will be driving for church purposes. This information will be used for the sole purpose of verification of statements made by you.)
I, the undersigned applicant, authorize the First Presbyterian Church of Saline through its independent contractor, Checkr, to procure MVR information about me. This report may include my driving history, including any traffic citations. Driver must obey all traffic laws especially speed limits, not using mobile phones while driving, mandatory use of seat belts for all passengers, etc.
I release First Presbyterian Church of Saline and any person or entity which provides information pursuant to this authorization, from any and all liabilities, claims or lawsuits in regard to the information obtained from any and all of the above referenced sources used.
When the MVR background check process begins a Checkr email invitation link will be sent to the email address provided below. It will include entering personally identifiable information and the following:
1. Your current, valid driver’s license number is required
2. A Summary of Your Rights Under the Fair Credit Reporting Act
3. Disclosure Regarding Background Investigation
4. Consent form: Candidate provides consent to the MVR background check process.
