By signing this form you are accepting responsibility for this method of building access and promising that you will not share/lend/provide your method of access to anyone else, but that it will always be within your control. If you open a door for someone else, you are responsible for their usage. Digital lock access will indicate your individual ProxCard, eBadge, or code opening the lock.

When you no longer serve in the above role, you are responsible for returning any ProxCard or key to the church office to be reassigned to someone else. If you do not return it, there will be a $10 fee. If your ProxCard or eBadge is lost or stolen, please report it immediately to a staff member or our Tech Coordinator so that we can take appropriate action to disable access.
